Dental Insurance Information
Dr. Kush Patel and Dr. Krutav Patel are participating providers with the following dental insurance PPO plans: Delta Dental, Cigna, United Concordia, Aetna, Anthem, Assurant, Guardian, Principal. For all others we will file as an “out of network provider”. For specific questions about your dental insurance benefits and out of pocket costs, contact our office and we will assist you.
